How Can We Help
At 360CCM, we offer chronic care management services that bridge the gap between the traditional monthly doctor's office visit and the convenience of patient care at home, without needing an in-person visit. Patients can select between video or phone appointments, whether they're at home or elsewhere. Our services include personalized care plans, medication oversight, remote patient monitoring, and additional support.

Your CCM provides managed care service between a traditional Doctor’s office visit each month. Patients can choose to connect with health professional via Telehealth. Your CCM provide services that include personalized care plans, medication management, remote patient monitoring, and much more.
We bill the same way as your Doctor’s office, through Medicare Part B or Veteran Community Care. If you do not have insurance we accept MC/Visa Discover option. If you have a co-pay that will be determined at enrollment.
We have an online portal that you can use to see your care-plan and pay for your services, or we can send you an invoice by mail or email.
